This information must be completed for each individual site you wish to gain pricing for. If you have two meters at one site and two supply numbers appear on your electricity bill, you must complete the form for each meter.

All 21 digits must be provided fot the supply number. The supply number can be found on your eletricity bill and begins with the letter S. If this information does not appear on your electricity bill you can request it from your local electricity board.
